Mountain-Pacific Quality Health (Mountain-Pacific) is part of a network of quality improvement organizations that operate locally under the direction of the Centers for Medicare & Medicaid Services (CMS).  Mountain-Pacific is a quality innovation network-quality improvement organization (QIN-QIO) serving Montana, Wyoming, Hawaii and Alaska.

Heart disease and stroke are the first and fourth leading causes of death in the United States. Mountain-Pacific Quality Health is aligning with the Million Hearts® initiative efforts by concentrating on the ABCS of cardiac risk reduction to prevent one million heart attacks and strokes by 2017 by:

Million Hearts® is a national initiative launched by the Department of Health and Human Services. The initiative brings together

communities, health systems, nonprofit organizations, federal agencies and private-sector partners across the country to fight heart disease and stroke.

Are You Ready to Take Charge of Your Diabetes?

Living with diabetes can be hard.

And left unmanaged, diabetes can lead to other health problems. The good news? Many people with diabetes, pre-diabetes or at risk for diabetes can live happy, healthy lives. Peer-led educational programs in your community can help you — and we can help you find them.

You probably know the phrase “knowledge is power.” When it comes to managing diabetes, this is definitely true. Mountain-Pacific is working in your community to help develop FREE, peer-led Diabetes Self-Management Education (DSME) courses. In these classes, you’ll learn how to:

Prevent serious complications and illnesses that can happen if you don’t manage your diabetes

Improve your eating habits

Increase your physical activity

Develop skills to take care of yourself — physically and emotionally

Improve your relationships with your health care providers, so you better understand one another

Coordination of Care

There have been significant gains in reducing avoidable readmissions and admissions in hospitals, but care coordination does not begin or end with an inpatient hospital stay. People are often unaware of the services available in their communities that can help them maintain their health. Mountain-Pacific recognizes the most effective means for improving patient care is through care coordination that opens lines of communications between the patient, providers and community support services. We help build relationships to improve the coordination of care and improve medication safety and reduce adverse drug events in local communities.
